T.W. is a 22-year-old man who fell 50 feet from a chairlift while skiing and landed on hard-packed snow. He
is now at the emergency department (ED) with a suspected T5-T6 fracture with paraplegia.
Describe a plan for implementing these physician's orders.
ED interventions for the primary survey of a trauma patient who is suspected of having a spinal
cord injury (SCI) assessing the ABCD (Airway, Breathing, Circulation, Disability). With this system, the
first action is to ensure that the cervical spine is immobilized, then initiate oxygen at 4 L per nasal
cannula. Place the patient on an ECG monitor and initiate two large bore intravenous (IV) lines.
Perform the neurologic assessment. Apply the warming blankets if the temperature is low because
some SCI patients exhibit poikilothermy, in which they take on the temperature of the environment
because of loss of autonomic function. Finally, insert a Foley catheter.
