What methods are available for removing a judge who cannot be considered impartial?

In most jurisdictions, either the defense or the prosecution can seek to have a judge removed for cause. That is, the individual seeking the judge's removal will argue that the judge is biased for or against a particular party to the case, usually by filing a motion for a new judge. Strangely, though, the only person who can remove such a judge is usually the judge himself!
