The most common menstrual problem is painful menstruation, called dysmenorrhea. Symptoms of this may include cramping pains in the pelvic region, headaches, backaches, nausea, and a feeling of pressure and bloating in the pelvis. Dysmenorrhea is caused by prostaglandins, hormonelike substances produced by many tissues of the body, including the lining of the uterus. These prostaglandins can cause smooth muscles to contract and can affect the size of the blood vessels. Women with severe menstrual pain have unusually high levels of prostaglandins which cause intense uterine contractions and choke off some of the supply of oxygen-carrying blood to the uterus. Prostaglandins may also cause greater sensitivity in nerve endings. The combination of the uterine contractions, lack of oxygen, and heightened nerve sensitivity produces menstrual cramps.
