If the vaginal canal is too tight for the infant to pass easily through it within a short period of time, an incision,called an episiotomy, is often made at the edge of the vaginal opening at an angle toward the ischium, and away from the anal opening. In theory, this incision will prevent a tear of the perineum, which might cause damage to the anal opening. However, often this procedure is not necessary and many physicians are, at the insistence of women they treat, using this procedure only if necessary. In previous times, this procedure was often done routinely whether or not it was needed.
