The licensed practical/vocational nurse (LP/VN) is caring for a client who is frequently admitted to the hospital with chronic renal failure

In the past, the client wanted his family and neighbors to visit freely and welcomed the nursing staff to share information about the client's health. The LP/VN is approached by the client's neighbor in the lobby of the hospital asking, "Could you tell me where Mary Smith is and what is going on with her?" Which is the best response to make to the client's neighbor?
A) "Let me check to see what room your neighbor is in and her visitor status."
B) "Oh, sure I am caring for her; she is in room 1203."
C) "You will need to check with Mary's family; she is not on my unit."
D) "She is in room 1203, and she is having problems with her dialysis access."

Ans: A
Feedback:
Privacy rules are the primary focus of the Health Insurance Portability and Accountability Act (HIPPAA), and just because the client in past admissions wanted her neighbors and family to know where she was in the hospital and update them on the information does not mean that she wants her private information handled the same way this admission. Every admission the client has to sign paperwork regarding the HIPPAA. The nurse's best response is to check the computer record and find out if the client still wants visitors and how to handle the private information. Providing the room number without checking first is risky because if the client changed how she wanted to deal with private information and did not want visitors or certain visitors, or information provided, then the nurse violated her rights. Stating that the client is not on the unit is a lie and is unethical.
