To obtain an endocervical smear, you should insert the Cytobrush through the speculum into the cervical os approximately 1 cm and
a. leave the Cytobrush in the cervical os for 30 seconds to ensure saturation.

d. swab the endocervix in a forward and backward brushing motion.

Using your dominant hand, insert the Cytobrush or cervical broom through the speculum into the cervical os approximately 1 cm. Many patients find that this procedure causes a cramping sensation, so forewarn your patient that she may feel discomfort during this element of the assessment. Rotate the Cytobrush between your index finger and thumb 360° clockwise, then counterclockwise. Keep the Cytobrush in contact with the cervical tissue. If you have to use a cotton-tipped applicator instead of a Cytobrush, leave the applicator in the cervical os for 30 seconds. If you use a cervical broom, rotate the broom 6 times clockwise. Do not use a forward and backward motion.
