While examining the hips of a 1-month-old baby, you hear an audible clunk as you abduct the left leg and again when you adduct the same leg. This finding is

a. associated with metatarsus varus.
b. Gower's sign associated with muscular dystrophy.
c. normal in infants under 12 months.
d. Ortolani's sign associated with dislocation of the hip.

D
Ortolani's maneuver is always performed at the very end of the assessment because it may produce crying. The Ortolani maneuver reduces a posteriorly dislocated hip. It is done to reduce a recently dislocated hip and is not done forcefully. In a positive test, the nurse senses reduction by a palpable, nearly audible "clunk." At about 6 to 10 weeks of age, Ortolani's sign disappears. Metatarsus varus is a medial forefoot malalignment. Gower's sign involves the inability to rise from a sitting to a standing position and is associated with muscular dystrophy.
