Histologically, in gingivitis, once dental plaque and calculus are removed, the pocket epithelium will reform into junctional epithelium and reattach to the adjacent tooth surface as it was in health. The inflammatory cells in the gingival connective tissue are replaced by collagen; this improvement results in an apparent "gain in clinical attachment."
Clinically, there is a reduction in or elimination of inflammation (e.g., bleeding) and the elimination of edema, both of which result in tissue shrinkage and occasionally gingival recession. Gingiva that is fibrotic will not shrink, as does edematous tissue. The gingiva often returns to its salmon pink color. The greatest reduction in probing depths occurs after 4–6 weeks and is primarily due to (a) tissue shrinkage, which may result in gingival recession, and (b) reformation of collagen, which results in greater resistance to probing. The reductions obtained in probing depths as a result of nonsurgical therapy are in the range of 1–2 mm.
