Women whose blood glucose levels are well controlled through diet are at low risk for fetal
complications. These complications include stillbirth, spontaneous abortion, hydramnios,
macrosomia, congenital anomalies, neonatal hypoglycemia and hyperbilirubinemia, and fetal or
neonatal death. Although no standard recommendations have been formulated for fetal surveillance
in pregnancies complicated by GDM, testing to identify fetal compromise can begin as early as
32 weeks of gestation, particularly if the woman has poor glycemic control. Surveillance testing
often includes "kick counts," ultrasonography for fetal growth and amniotic fluid volume, biophysical
profile, nonstress test, contraction stress test, or amniocentesis for fetal lung maturity.
