• First, divide the history information into critical information and "nice-to-know" information;
address the former first.
• Determine the time and date of her last bowel movement, time of her last urination, color and
amount of stool and urine, and when she last ate. Is there any history of cardiac, pulmonary, or GI
problems?
• Ask about medication allergies.
• Double-check medications. Remember, U.S. brand names that you are familiar with will be
considerably different from drug names used elsewhere. Patients rarely are familiar with generic
names. In addition, the drugs often have a different appearance (e.g., color, size, shape). For a
foreign drug resource, check the poison control center.
• Ask detailed questions about her pain using the mnemonic COLDERRA: Characteristics, Onset,
Location, Duration, Exacerbation, Radiation, Relief, and Associated S/S.
