Sickled RBCs are friable and rupture more easily than normal RBCs, causing the hemoglobin and
hematocrit values to decline. Vascular occlusions occurring in the kidneys lead to renal failure and
result in decreased erythropoietin production. Because erythropoietin is not stimulating the bone
marrow to produce RBCs, the hemoglobin declines even further.
