A complaint includes the following sections:
Captionâ€"The heading, which identifies the name of the court, the title of the action, the names of the parties, designation of the document as a "complaint," and space for the docket number to be inserted.
Jurisdictional allegationsâ€"Assertions proving that the court has the jurisdiction (or authority) to hear and decide the case.
General allegationsâ€"The body of the complaint chronologically outlining the events giving rise to the plaintiff's claim(s).
Prayer for reliefâ€"A paragraph at the end of the complaint, with which the plaintiff requests that the court enter a judgment for the plaintiff and grant appropriate relief, such as money damages.
Signatureâ€"A line following the prayer for relief where the attorney signs his name to indicate that he has read the complaint and that the facts alleged are true to the best of his knowledge.
Demand for jury trialâ€"A request by the plaintiff for a jury trial.
