• Is this provider accepting new patients?
• What insurance plans does this office accept?
• If a physician, is this doctor board-certified, meaning that he or she has undergone extra training after medical school to specialize in an area such as family practice?
• How does the office handle lab work? Is there a lab in-house or nearby, or will you have to travel to a different location for a procedure such as a blood test?
• Is this a group practice? If so, will you mostly see your provider or all the providers in the group?
• Is this medical practice affiliated with any particular hospitals or specialty centers?
• Does this practice offer newer methods of contacting your provider such as email?
