A 3-day-old preterm infant is diagnosed with physiologic jaundice. When subjecting the newborn to phototherapy, to which of the following must the nurse pay attention?
A) Encouraging frequent maternal–child bonding
B) Avoiding frequent feeding of the newborn
C) Wrapping the infant in warm clothes
D) Preventing hypothermia or hyperthermia
D
Feedback:
When subjecting the newborn to phototherapy, the nurse must pay attention to prevention of hypothermia or hyperthermia. Phototherapy is the use of ultraviolet (UV) light from sunshine or intense fluorescent light to alleviate symptoms of jaundice by accelerating the elimination of bilirubin in the skin (photo-oxidation). During the procedure, the vital signs should be frequently monitored to alert for impending hypothermia or hyperthermia. Generally, the baby is placed on a 3-hour feeding schedule because frequent feedings help to speed the excretion of bilirubin. The infant should only wear a diaper and not be wrapped in clothes during phototherapy, to allow maximum exposure of the skin to UV light. Supplemental water is necessary to prevent dehydration. The infant may be handed over to the mother for maternal–child bonding after the procedure is complete.
