What kinds of assessment should Emelia try to arrange for the Snyders?

Emelia coordinated a Meals on Wheels Program for the elderly, and supervised 10 volunteers who delivered the meals. One day, when Emelia filled in for a volunteer and delivered a meal to the Snyders, she found Mr. Snyder very agitated and worried because Mrs. Snyder was missing. Apparently she had wandered off before. Because it was cold outside and they lived two blocks from a busy street, Emelia was alarmed and struggled with how to respond to this unexpected situation. As she tried to prioritize her responsibilities, she began to wonder if her volunteers were communicating with her and if they were well-trained for their responsibilities.

It would be a wise idea to make sure Zelda has a thorough assessment by a gerontological specialist to determine if there is a condition that could be causing her to wander and whether it can be alleviated with treatment. Jim also needs a comprehensive assessment to determine how well he is managing his diabetes, what is causing his weight loss, and whether his level of agitation is problematic.
Also Emelia needs to find out more about the family and whether there are family members or friends or neighbors who could provide regular social support.
The Snyders may be eligible for coverage of the cost of the evaluation through a combination of Medicare and Medicaid. If not, many state funded hospitals have a program to cover costs of indigent care. A hospital social worker should be able to go over these programs with Emelia. The Snyders will need assistance in scheduling the necessary appointments and in arranging transportation. Perhaps Mr. Snyder?s diabetes could be evaluated at the same time. It is possible that the Snyders will need to have housing arranged for a visit of a couple of days if they must go out of town for the evaluation.
