Painful adnexal masses are abnormal. In early pregnancy, these can indicate an ectopic pregnancy (pregnancy other than intrauterine, such as in the abdomen or fallopian tube), infection, or cancerous growth. Pain associated with an adnexal mass can be elicited via cervical motion during bimanual assessment. Abruptio placenta and pregnancy-induced hypertension (PIH) cause persistent abdominal pain or tenderness. Abruptio pain is from retroplacental bleeding of the placental separation. PIH pain is secondary to the hepatic involvement.
