First and foremost, any pertinent case records must be made available to the CDR team for the review to be conducted. Secondly, the appropriate members must be in attendance, and there must be an adequate level of trust between the members. The National Center for the Review and Prevention of Child Deaths goes into detail regarding the different types of deaths, but some issues are present regardless. Issues to be explored and reviewed include facts, reports, risk factors, services for the family, community, surviving family members and caregivers, agency practices, and preventative actions. The reason it is important to have such a comprehensive review of related services, facts, family members,and so on, is to determine if there were missed opportunities to identify harm by any service agency. Focusing on the systems and making recommendations based on findings can lead to significant outcomes.
