The cap must be on the Lukens tube or there will not be sufficient suction generated to move the sputum into the tube. The sputum is heavier than air. If the suction catheter were higher than the suction source, the sputum would be sucked out of the tube through the lower suction source tube. With the suction source attached higher on the tube than the suction catheter, any sputum entering the tube falls to the bottom of the tube.
