S = Subjective: Anything the patient reports but cannot be observed by the provider; e.g., pain, numbness, headache, nausea, etc.

O = Objective: Anything that the provider can observe; e.g., twitching, limp, vital sign readings, such as blood pressure, pulse, temperature, respiratory rate, etc.

A = Assessment: The conclusion based on the subjective and objective information; e.g., any diagnosis.

P = Plan: What is going to be done to treat the diagnostic findings; e.g., any treatments or medications prescribed or plans for further tests, etc.
