You inform the physician of J.S.'s assessment. The physician believes J.S. has developed
Cushing syndrome and decides to change his prescription from dexamethasone (Decadron)
daily to prednisone (Deltasone) given on alternate days.
Explain the rationale for this change.
Administering steroids on alternate days may decrease side effects and diminish the degree of
hypothalamic-pituitary-adrenal suppression. Dexamethasone causes adrenal suppression lasting
2 days or more and would not be suitable for alternate-day therapy. The adrenal suppression
associated with prednisone is shorter, lasting 1 to 1½ days.
