This model and just about all of the supporting research/theory associated with it was developed using the DSM-IV abuse/dependence diagnostic criteria with dependence defined as the “severe” substance use diagnosis. The development of the mild, moderate, and severe use disorder criteria for the DSM-5 substance use disorder diagnosis likely forces a change to this model. At face value, it appears that the quadrant model needs to expand. For example, there are now three levels of substance use disorder (mild, moderate, and severe). There are also multiple levels of the other disorders, such as mild, moderate, and severe major depressive disorder. A 3x3 model seems more applicable to the DSM-5 diagnostic structure.
