Postoperative care of the client is essential for providing comfort and preventing complications. Additional assessment includes monitoring vital signs, drainage from drains, tubes, wounds for signs of infection, hemorrhage, or fistula formation. Immediately report any evidence of increasing abdominal distention, shock, bloody stools, hematemesis, or bleeding from incisions or drainage tube sites. Monitor color, consistency, and amount of drainage from NG or gastrostomy tubes. Maintain tube patency by preventing dislodgement and kinks in tubing; maintain suction as ordered by physician. Do not irrigate tubes unless ordered by physician. Post surgery, ambulation and activity are encouraged as tolerated to prevent postoperative complications of thrombophlebitis, ileus, obstruction, respiratory infection, pneumonia, and atelectasis.
