A client has experienced a severe sunburn. The nurse is aware that is an example of which of these burn classifications?

a. first-degree c. third-degree
b. second-degree d. fourth-degree

A
The severity of burns is classified according to the depth and extent of skin surface involved. First-degree burns affect the epidermis only; the skin appears hot and red and is usually painful (e.g., sunburn). Second-degree burns involve the dermis and epidermis, in which blister formation occurs and the skin is edematous and painful (e.g., scalding by a hot liquid such as water or coffee). First- and second-degree burns are partial-thickness burns. If the dermis and epidermis are completely destroyed by the burn, it is classified as a full-thickness burn. Third- and fourth-degree burns are full-thickness burns that are not painful because sensory nerve endings have been destroyed.
