Substantivity is defined as the ability of the drug to adsorb or bind to intraoral surfaces such as teeth and soft tissues with subsequent release of the drug in its active form. Substantivity involves the ability of the drug to stay at the target site for longer periods of time, thus maintaining therapeutic levels. It is ideal to have a drug with high substantivity that will be bound in the oral cavity and released over a period of hours in order to prolong the effects. Lack of substantivity can be overcome by more frequent use of the agent, but this would likely result in noncompliance and undesirable side effects. Chlorhexidine has high substantivity and Listerine has low substantivity.
