The EOB is an "Explanation of Benefits," which is a document generated by the insurance company explaining how the reimbursement was determined. Both the patient and the provider receive an EOB. The provider's EOB is a summary of all benefits paid to the provider. The patient's EOB summarizes how the insurance company determined the reimbursement for the services the patient received. The reverse side of the patient's EOB often has answers to commonly asked questions about the EOB, an antifraud hotline number, and definitions for terms used in the EOB.
