As a behavioral health care specialist, the social worker can assess how behavioral needs might interfere with the management of physical needs and monitoring of health care. For instance, a referral from primary care may result in social worker making a home visit to assess the person’s capacity to manage household tasks and perhaps refer to home health care services. The social worker in this role would assess the emotional state that may interfere with adherence to prescriptions given by the primary care provider to manage health concerns. The social worker addresses the emotional state, either through direct support or referral for psychotropic medications that enhance the person’s ability to follow the medical protocols and stay out of the hospital.
