When examining your patient, who has a thiamine deficiency related to chronic alcoholism, you are most likely to observe evidence of cranial nerve (CN) involvement resulting in the inability of his eyes to move

a. downward (CN IV). c. outward (CN VI).
b. medially (CN III). d. upward (CN III).

C
Vitamin deficiency, especially thiamine (which can occur in chronic alcoholism), can result in the inability of the eyes to move outward. Cranial nerve VI, the abducens nerve, innervates the lateral rectus muscle and allows the eye to move outward. Therefore, this patient may have involvement of CN VI. CN IV, the trochlear nerve, innervates the superior oblique muscle, allowing the eye to move downward when deviated inward. CN III, the oculomotor nerve, innervates the superior, inferior, and medial rectus and the inferior oblique muscles. CN III allows the eye to move medially and upward.
