A 30-year-old man was hospitalized for hemoptysis. Kidney function was compromised
In the kidney biopsy, most of the glomeruli resembled the one illustrated here and the immunofluorescence microscopic examination revealed linear staining of the glomerular basement membranes with the antibodies to IgG.
What is the diagnosis?
A. Wegener's granulomatosis
B. Polyarteritis nodosa
C. Goodpasture's syndrome
D. focal glomerulosclerosis with hyalinosis
E. acute poststreptococcal glomerulonephritis
C
Crescentic glomerulonephritis with a linear staining of glomerular basement membranes seen on immunofluorescence microscopy is diagnostic of Goodpasture's syndrome. In acute poststreptococcal glomerulonephritis, there are granular deposits along the basement membranes and in the mesangial areas. The glomerulus in patients who have Wegener's granulomatosis or polyarteritis nodosa does not show linear staining with the antibodies to IgG, and in most instances is unreactive with any antibodies except for some minor focal staining; hence the term "pauci-immune glomerulonephritis." Focal glomerulosclerosis may show segmental trapping of IgM.
