ANSWER: The Apgar scale is used at one minute and five minutes after birth to assess the neonate’s readiness to be outside the womb. Vital signs such as heart rate, skin color, muscle tone, reflexes, and breathing are each rated on a scale of 0 to 2. A score of 4 or less indicates the baby needs immediate help.
