Tom is a 42-year-old black male who presents to the ED with vision problems. He states that when he woke up this morning, he noticed that the vision in his left eye was "funky." He describes it like "having a dark shadow across the lower portion of my eye." He had no visual disturbances when he retired the night before, and denies any vision problems previously. He does not wear glasses for
distance, though he acknowledged he would probably need reading glasses "soon." He denies any history of hypertension, diabetes or high cholesterol. He is very physically fit and says he watches his diet and exercises "religiously." A neuro-ophthalmologist was called to consult and she diagnosed Tom with ischemic optic neuropathy. She prescribed large doses of steroids and told Tom there was a good chance his vision would improve or return entirely. She told Tom to follow-up with her in one week.
