What will be an ideal response?


1) Statement of child's level of development.
Cognitive, communicative, physical, adaptive, and social / emotional are identified based on a multidisciplinary assessment

2) Statement of family priorities, resources, and concerns.
Identified as they relate to the development of the infant or toddler to enhance family involvement in early intervention
3) Major outcomes.
Must have criteria for mastery, a timeline for implementation, an expected date of completion, and a discussion on the procedures for measuring progress
Can focus on family or child

4) Statement of early intervention services.
Must include the frequency, duration, location, and method of delivery of the service

5) Projected dates for initiation and duration of services.
Generally six months (which coincides with reviewing)
If longer than six months, must work with the family to establish a timeline for the initiation, delivery, and completion of those services

6) Service coordination.
Early intervention services may be provided by more than one agency and therefore the IFSP must have a service coordinator.

7) Transition plan.
Must be in place at least six months prior to the child transitioning to early childhood special education preschool services
May include visits to the preschool classroom, a visit with the early childhood special education teacher, a multidisciplinary assessment to transition the child from an IFSP to an IEP, an IEP eligibility meeting, and educating the parents and family to the legal and programmatic changes that can be expected
