S: The subjective section that describes what the patient reports about his/her diet but the clinician does not directly observe. This includes the medical, dental, diet, and psychosocial history.
O: The Objective section that describes clinical information that is observed or obtained by the clinician. This includes the use of any risk assessment tools, the clinical examination, and the taking of a diet record.
A: The assessment section includes the synthesis of the findings into a diagnosis based on the subjective and objective data collected. The diet is analyzed for overall adequacy and cariogenic risk.
P: The plan section includes a treatment plan for the patient created based on the information obtained in the first three sections. The plan should address the patient's goals as well as any needed consultations or referrals. Goals should be specific and measurable, and decided along with input from the patient.
