When varicosities are present in the legs, use the retrograde filling or Trendelenburg test to determine valve competence in the communicating and saphenous veins. This test is performed by placing the patient in a supine position and raising the involved leg 90° to drain the venous blood. Next, place a tourniquet high on the thigh to occlude the saphenous vein. Instruct the patient to stand so that venous filling can be noted. Release the compression and watch venous filling again. Normal saphenous veins fill from below the compression and within 35 seconds as blood flows down from the arteries to the veins. Following release of the tourniquet, there should be no additional filling because competent valves prevent the backflow of blood.
