Individual factors- things individuals are primarily responsible for, including food and nutrient intake and nutritional adequacy, physical activity and other lifestyle habits' and the results of individual behavior on weight status and chronic disease risk factors. These are monitored at the individual level using standard indicators, summarized across population groups, and compared to national Healthy People 2020 objectives.
External or environmental factors- things that are the result of decisions made by planners, retailers, employers and healthcare providers and others that affect the environments in schools, worksites, and the community; as well as the social environment (support systems, income, education, discrimination), physical environment (housing, transportation, location of retail stores and restaurants, and availability and quality of health and nutrition services. These are monitored by the local community, state and federal levels and compared to HP 2020 objectives.
