Which neurologic interventions could you delegate to the nursing assistive personnel (NAP)?

Select all that apply.

a. Reminding S.B. not to cough or sneeze
b. Assessing his pupillary response to light
c. Noting S.B.'s intake and output with each incidence
d. Assisting S.B. to sit in the bed with the head elevated
e. Evaluating S.B.'s response to his as-needed pain medications
f. Instructing S.B. on the importance of deep breathing every hour

a, c, d
The NAP cannot instruct the patient on the importance of deep breathing; teaching is the
responsibility of the registered nurse (RN). However, the NAP could reinforce teaching provided by
the RN, including reminders not to cough, sneeze, or blow the nose and the importance of deep
breathing. All providers would be responsible for assisting S.B. in staying in bed with the head
elevated. The NAP can document intake and output as long as the RN has given the NAP parameters
that the NAP would need to report to the RN. Assessing pupil reaction and evaluating response to
pain medications are not within the scope of practice for the NAP.
