Why should psychologists document unexpected contacts with clients?

What will be an ideal response?

For some psychologists who work under HIPAA, it is necessary to document every contact with the client, including unexpected contacts with clients outside of the office, or by phone, or by the Internet. Further, documenting unexpected contacts with clients, as well as clients’ family, other professionals, and partners may be necessary to ensure adequate service or protection of the client’s welfare.
