Such follow-up efforts can reveal whether the care plan needs to be revised or updated, as is often the case when a person's medical condition or situation changes. (For example, after a woman delivers a baby, she may need instructions on how to feed her infant or, if she is breastfeeding, how to modify her diet to support lactation). If a follow-up meeting with a dietitian is not possible, a dietetic technician or other qualified health practitioner should provide additional guidance and education.
