Discuss how the "health continuum" could be utilized when planning health promotion


The "health continuum" depicts health as a continuum from a well population to a population at risk to a population with an established disease to one with a controlled chronic disease. Health promotion is more effective when the programming is targeted to the level of health within the population. Primary prevention activities are best for those who are well whereas tertiary prevention activities are more appropriate for those with an established chronic disease.
